Low Performance Flags

Low performance flags are intended to be used as a tracking system that highlights both occasional incidents and repeating patterns of sub-standard performance among your trainees.

When a trainee receives a score on an evaluation that is below a pre-set value (or "low performance flag"), an electronic discussion is initiated between educators and administrators to determine whether or not further action should be taken. Further steps might include scheduling a one-on-one meeting with the trainee, waiting for results from other evaluations and, in rare cases, disciplinary action.

Discussions of low performance incidents remain private between the individuals involved. The trainee is not included in the process unless it is deemed necessry to do so. However, one45 stores a full record of each low performance incident, including all comments, dates and the individuals who were involved in the case, to ensure that proper follow-up takes place.

Incidents that are open (i.e., still being reviewed) are stored in the active flags list. Incidents that are closed (i.e., everyone involved has signed off on the case) are stored in the flag archive.

As the administrator for your program, you set the low performance flags on all of your evaluation forms. Deciding where to set the flags (i.e., deciding what constitutes a low performance) will likely require input from a number of administrators and faculty members. These decisions will be based not only on the particular evaluation forms and their components but also on the overall educational philosophy of your program. In addition to setting the flags, you also decide who should be notified when a low performance incident occurs. These individuals will receive a new task in their To Do inbox every time a low score is registered. It is then their responsibility to review the case, add any comments they have and invite others into the discussion as they see fit. An incident is only considered closed once everyone who has been invited to comment has signed off on the case.
