The Low Performance Flags Archive

When everyone who has received notification of a low performace flag has signed off on the case, the incident is considered closed. The details of the case (i.e., all comments that were made, the individuals that were involved, the dates) are then stored in the flag archive. This archive is intended to provide an official record of all low performance incidents in your program, and you can visit it at any time to review a closed case.

To review a closed low performance incident:

1 Click the Forms tab.

2 Click "Low Performance."

3 Click the Archived Flags tab. A list of all the closed low performance incidents in your program appears.

The archived flags list consists of the name of the trainee, their academic level, the rotation on which they achieved a low performance score and the date range of this rotation. You can sort the archive in different ways by clicking on the appropriate header (Name, Level, Rotation or Date). By default, the archive is sorted alphabetically by trainee first name.

You can view the entire history of each flag by clicking the arrow beside the trainee's name.

The case details are arranged chronologically from top to bottom. You can see all of the comments that were made, including the dates of each comment and which individuals were involved (people who were forwarded the case appear in italics).

You can also see the original evaluation form on which the trainee received a low performance score by clicking "View form result." A new window will appear displaying the evaluation.
