A low performance incident is considered open, or active, until everyone who was notified of it has signed off on the case. One45 keeps a list of the active flags in your program so you can quickly see which flags still require follow-up.

To view your active low performance flags:
1 Click the Forms tab.
2 Click "Low Performance." The Active Flags tab appears.

The active flags list consists of the name of the trainee, their academic level, the rotation on which they achieved a low performance score and the date range of this rotation. You can sort this list in different ways by clicking on the appropriate header (Name, Level, Rotation or Date). By default, the list is sorted alphabetically by trainee first name.
| Low performance incidents are organized and stored in the system under the trainee's name. Therefore, if a trainee has more than one low performance flag they will appear more than once in the list. To differentiate between these cases, use the rotation and date information. |
You can view the history of each low performance incident by clicking the arrow beside the trainee's name.

The case details are arranged chronologically from top to bottom. You can see all of the comments that have been made so far, including the dates of each comment and which individuals have received notification of the flag (people who have been forwarded the case appear in italics). In this manner, you can quickly determine why the flag is still open (e.g., the incident is still being discussed, a faculty member hasn't viewed the details yet, etc.).
You can also see the original evaluation results by clicking "View form result." A new window will appear displaying the evaluation.
